Padan Fain/Mordeth will be sealed in the Dark One's prison with him for eternity, where they will battle amongst themselves for eternity. This is alluded to by the wound in Rand's side which could only be sealed off from Rand after Fain cut him.

Alanna disappeared at the end of TOM to someplace with Lanfear and while there she is going to transfer her bond with Rand to her. Later she is going to die alongside the Dragon in Tarmon Gaidon.

While Rand and his select band of channelers (Nynaeve, Narishma, etc.) attempt to seal the bore, Bashere and Itruralde will be the ones to lead the "dragon legion". Mat will follow after Rand. As Rand has his inevitable confrontation with Moridin, Shaidar Haran will attack Rand, trying to distract him or kill him. Mat will then have an epic duel with Shaidar Haran but will be outmatched in the end because of his missing eye (which is covered by a bad ass eye patch like Ingtar's). But just as Shaidar Haran is about to strike Mat down, Rand is triumphant and seals the bore. Yay!

The thirteen black towers in the TOM represent the chosen. I am not entirely sure which of them is represented by the cracked tower, but I think it could crazily be Moghedien (tho Lanfear would make more sense it would contradict my earlier predictions)

Rand will die, but just like the dead forsaken, he will survive in a new body. That body will be Ishamael's "new" body: Moridin.
His "old" body has been damaged by Ishamael, Padan Fain, and he only has one hand. The prophesy says he must die to live. Frankly he could use a new place to stay! Since he has a weird connection to Morridin already, he might as well take that one.
