Just a quick thought from the Guide - In the descriptions of the Male Forsaken, something sticks out about Ishy. At the end of the section, it says that Ish was killed in the Stone of Tear by Rand al'Thor yada yada yada. Then the last sentence says that the Dark One is the lord of the grave, or something like that. None of the other forsaken that are dead have this little after thought added on. To ME that says Ishy is back, even though his Black Cord was cut.
